Fill out the form below to receive your free water bottle for participating in Bike-to-Work Week! Only one person per household needs to fill out the form if you all took the same route. Please fill out separate forms if you took different routes. Friends and neighbors who took the same route should fill out separate forms. Parents may fill out a form for their children if the children participated.

The more optional questions that are filled out, the higher your chances are to win a free Bike Layne t-shirt. It is a sky blue synthetic fabric, great for biking! A water bottle is guaranteed for everyone, but there are only a few t-shirts available. They will be raffled off from the pool of participants with an extra entry per two (2) optional questions filled out.
